Every client has the right to:
Be informed prior to or at intake about:
Available services,
Required payments (if not covered by insurance).
Receive complete and current information about their diagnosis and treatment plan in understandable terms.
Expect courteous and respectful treatment.
Be informed of and refuse participation in experimental research.
Choose freely among mental health professionals and change providers (within insurance policy limits).
Assert their rights without retaliation.
Clients receiving services from practitioners licensed in Texas also have the right to:
Expect practitioners meet state-mandated training/experience requirements.
Examine public records of practitioner credentials.
Obtain a copy of the rules of conduct.
Marie Taniform, PMHNP-BC, is the sole practitioner at Sparkle Path Psychiatric Care.
She is a Board-certified Psychiatric Mental Health Nurse Practitioner licensed in:
Texas,
Washington,
New Mexico.
She collaborates with Linda Lefler, MD, and may consult on your care.
We are in-network with most insurers. As a courtesy, we bill your insurance directly.
Your responsibilities:
Verify your benefits, deductibles, and exclusions before your first appointment.
Provide your insurance card at intake and update us on any changes.
Pay co-pays, coinsurance, and deductibles at the time of service (estimates may adjust post-claim).
Full payment is due at the time of service.
We provide a superbill for potential reimbursement from your insurer.
Discounted rates may be available for financial hardship (ask for details).
Insurance verification is not a guarantee of payment.
Denied claims due to inaccurate information become the client’s responsibility.
Balances unpaid for 30+ days may result in service denial.
All balances remain due even after discharge.
Psychiatric diagnosis,
Medication management,
Limited individual psychotherapy (if available).
Involve a comprehensive evaluation and proposed treatment plan.
Note: Medications/therapy have benefits and risks; specific outcomes are not guaranteed.
Treats ages 6+.
Teens may consent independently to treatment (confidentiality applies).
Parental involvement:
General progress/attendance updates require teen’s verbal agreement.
Exceptions: Safety concerns (e.g., harm to self/others).
Clients must contact and evaluate referred providers.
Cancellation: Notify via Patient Portal or phone (voicemail) ≥48 hours in advance.
No-show fee: $75 per missed appointment.
3+ no-shows: May result in discharge from practice.
Email is not secure—use the Patient Portal for clinical messages.
Avoid email for:
Suicidal/homicidal thoughts,
Emergencies,
Severe side effects.
Emergencies: Call 911 or go to the nearest ER.
Phone calls: Leave a message; responses may take time.
Refill requests:
Have your pharmacy fax to 469-589-7511, OR
Leave a voicemail with:
Medication name/dose,
Pharmacy details.
Processing time:
Refills: 3 business days.
Prior authorizations: 5–7 business days.
Monthly appointments are required for refills.
Confidentiality: Records are released only with written consent (except for care coordination).
Fees:
First 3 pages: $25,
Additional pages: $15/page.
Processing time: 5 business days.
We accept:

Personal checks,
Credit/debit cards (Visa, Mastercard, Amex, Discover).
Note: Out-of-network services require full payment upfront.
